Janumet 50 500mg is a diabetes medicine combined with diet and exercise to control blood sugar levels in patients with type 2 diabetes. It is prescribed:
- As monotherapy for those who are not controlled with metformin treatment alone
- It is prescribed with sulfonylurea in patients whose diabetes is not controlled with Metformin
- With premixed long/intermediate-acting insulin.
- With pioglitazone, when patients are not controlled on Metformin and Pioglitazone.
Janumet combines two medicines, Sitagliptin and Metformin, which work together to lower blood sugar levels in adults with type 2 diabetes. Metformin effectively lowers the amount of sugar produced by the liver. Sitagliptin helps increase the insulin levels produced during high blood sugar, especially after a meal. The medicine effectively decreases the amount of sugar produced by the body. The combination helps to achieve better blood sugar control.
Precaution with Sitagliptin and Metformin
- Janumet 50 500 mg should not be taken by patients who are suffering from type 1 diabetes or for the treatment of diabetes ketoacidosis.
- Jamumet is contraindicated in patients with severe kidney disease, hypersensitivity to Metformin or Sitagliptin, or a history of serious hypersensitivity reactions to Janumet 50+500 mg.
- The combination drug should not be given to pediatric patients under 18.
- The consumption of alcohol during the treatment may potentiate the effect of Metformin. Therefore, patients should be warned against excessive consumption of alcohol while receiving diabetes medicine.
- Cases of pancreatitis, called inflammation of the pancreas, have been reported in patients using this medicine.
- If a patient receives skin blistering, it could indicate the development of a bullous pemphigoid condition.
- The medication may give rise to a very rare but serious condition known as lactic acidosis, particularly in the case of renal insufficiency. The risk usually increases with uncontrolled blood sugar, prolonged fasting or alcohol consumption, serious infection, dehydration, liver insufficiency, and or medical condition associated with a reduced supply of oxygen.
- Discontinue treatment and seek immediate medical attention if you develop some of the symptoms associated with lactic acidosis. The symptoms include stomach ache, vomiting, muscle cramps, a general feeling of not being well because of extreme tiredness, trouble breathing, and reduced body temperature and heartbeat.
Side effects of Sitagliptin and Metformin
The reported side effects of the combination drugs are sore throat, stuffy or runny nose, joint pain, muscle aches, headache, itching, blisters, joint pain, back pain, arm or leg pain, and gastrointestinal symptoms (diarrhea, nausea, constipation, upset stomach, loss of appetite, and abdominal bloating). These are not the possible side effects associated with using Sitagliptin/Metformin. If you develop any side effects that are not listed here, it is important to speak to the doctor.
When this combination is used with a sulfonylurea medicine or with insulin, there may be a sudden drop in blood sugar. Lower doses of the sulfonylurea medicine or insulin may be required when using the combination of Sitagliptin and Metformin. Janumet can cause abnormal blood test results. Doctors may advise blood tests before and during the treatment. They may also monitor a patient’s liver, vitamin B12 levels, thyroid, and kidney function.
How to use Sitagliptin and Metformin?
Doctors will individualize the starting dose of Janumet based on the current treatment regimen. It is always advised to take the medicine exactly as the doctor recommends. The healthcare provider will instruct a patient on the dosage and frequency of this combination medication. Dose adjustments can also be made, if necessary, to achieve further control over high blood sugar levels.
Typically, this combination medication is taken orally twice a day with meals to minimize the risk of gastrointestinal upset. The combination tablet should be swallowed whole and not split, broken, or chewed before swallowing. Continue to take the medication for as long as your doctor recommends to help control your blood sugar levels.
You may need to discontinue treatment for a short period. Talk to your healthcare professional if any of the following applies to you:
- Have health problems related to dehydration, such as being sick with severe diarrhea, vomiting, or fever, or if fluid consumption is too low.
- Plan to undergo a surgical procedure.
- Are going to receive an injection of dye for an X-ray procedure.
